In this per diem position, you'll conduct home health risk assessments across Lacey, Olympia, and Tumwater. These visits, averaging 45 minutes each, are essential in identifying patient needs and ensuring the highest standards of carewhether completing an annual health assessment or a post-discharge check-in.

Key Responsibilities:

Perform thorough home health risk assessments for Medicare Advantage health plan members Review and document prior and current medical history, existing medical conditions, and medication regimens Educate patients on their health conditions and the effective management of prescribed medications Identify and communicate gaps in care, offering practical recommendations for patient well-being

Qualifications:
