  • Home Health Job Summary Provides skilled nursing care to patients in their homes, performing assessments, treatments and care coordination across an assigned caseload.
Develops and updates the plan of care in collaboration with the physician and interdisciplinary team. Educates patients and caregivers to support safe self-management and independence in the home setting. Education & Experience Graduate of an accredited school of nursing: Required Home health, community or acute care nursing experience: Preferred Licenses & Certifications Registered Nurse licensure in the state of practice: Required Basic Life Support (BLS) certification and a valid driver's license: Required Key Responsibilities Conduct comprehensive admission and ongoing assessments in the patient's home. Develop, implement and revise the plan of care in collaboration with the physician. Perform skilled nursing interventions, including wound care, medication management and IV therapy. Educate patients and caregivers on disease management, medications and safety in the home. Coordinate care with therapy, social work, home health aides and community resources. Complete timely, accurate documentation and participate in on-call coverage as scheduled. #Workwolf
