  • Long Term Care Job Summary Provides direct personal care and activities of daily living support to residents in a long-term care setting under the supervision of a licensed nurse.
Assists residents with bathing, dressing, grooming, toileting, mobility and nutrition while promoting dignity, independence and safety. Observes and reports changes in resident condition and documents care provided. Education & Experience High School Diploma or equivalent: Required Prior experience in a long-term care or skilled nursing setting: Preferred Licenses & Certifications Certified Nursing Assistant certification in the state of practice: Required Basic Life Support (BLS) certification: Required Key Responsibilities Assist residents with activities of daily living, including bathing, dressing, grooming, toileting and mobility. Assist with meal service, feeding and hydration, and document intake and output as required. Take and record vital signs, weights and other measurements as directed by the licensed nurse. Observe residents for changes in physical or behavioral condition and report findings promptly to the charge nurse. Use proper transfer, lifting and positioning techniques to maintain resident and staff safety. Document care provided in the resident record and follow infection control and safety policies. #Workwolf
