Your Role:
Shaping Exceptional Care As a Nurse Manager, you'll be a critical leader in ensuring our residents receive the highest quality of care.Your key responsibilities include:
MDS Assessments:
Complete comprehensive and accurate MDS assessments in compliance with federal and state regulations.Resident Care:
Conduct thorough assessments of residents' physical, psychological, and social needs, utilizing the MDS tool to document their status effectively.Care Planning:
Develop and maintain individualized care plans based on assessment data. Lead interdisciplinary care plan meetings to ensure collaborative and effective care strategies.Compliance Excellence:
Ensure all practices meet CMS guidelines, state regulations, and industry standards.Team Collaboration:
Partner with physicians, therapists, dietitians, social workers, and other healthcare professionals to provide holistic and coordinated care for residents. What You Bring to theTeam:
Active RN/LPN License:
State licensure inWI Experience Matters:
At least 2+ years in a healthcare facility or related setting. Expertise inCare:
A strong understanding of skilled nursing standards and best practices.Critical Thinking:
