What Youll Do:

Complete comprehensive home health risk assessments for Medicare Advantage plan members Conduct detailed evaluations of patient medical history, current conditions, and medication usage Provide patient education regarding medical conditions and prescribed medications Identify care gaps and recommend solutions, ensuring improved patient health outcomes Focus solely on assessmentno treatment plan creation or medication prescribing

What We Look For:

Active Nurse Practitioner license in Wisconsin Current BLS (CPR) certification Board certified through AANP or ANCC Minimum of 6 months current NP experience (preferred) Master's degree in Nursing from an accredited program This per diem role offers compensation of $80$90 per completed evaluation, along with a mileage stipend and administrative time reimbursement.
