This position is for 8-16 hours per week. Job Description Provide planning and delivery of direct and indirect patient care through the nursing process of assessment, planning, prevention, and evaluation. Work closely with management to ensure we are providing excellent resident care. Delegate care aides to pass medications Assist in assessments of new residents as needed Do weekly med cart audits Review of medication administration records Provide health monitoring and follow up for each resident upon an envent, incident, or change in condition Assist in investigations Ensure weekly resident reviews are occurring with management Review of quarterly psychotropic medication Review of monthly psychotropic medication Review charting on a regular basis Assist with training care aides on nurse specific tasks. Prepare and administer medications as ordered by physician and perform other drug administration. Assist in care plan meetings as needed Update residents' doctors and family as required, as needed. Assist in updating residents' assessments and ISP's as needed Regular and continuous monitoring of wound care needs Communicate changes in patient clinical condition with physician, nursing supervisor and family Ensure that the community is in compliance and meets the standards of DHA 83 and Touchstone Senior Services. Assist in completing required record keeping upon residents' admission, discharge, transfer; chart and document per facility policy Requirements Graduate from an accredited nursing program. Hold a Current State License as a Registered Nurse Able to communicate well in both verbal and written forms Have a minimum of 1 year experience as an R.N. (assisted living facility experience preferred but not required) Commitment to our staff, residents, and Sylvan family.
