Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage plan members Obtain and evaluate prior and current medical histories, ongoing medical conditions, and medication usage Educate patients on their medical conditions and medication regimens Identify care gaps and offer actionable recommendations for improved patient outcomes Visits may include annual health assessments or post-discharge check-ups No requirements for creating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in WV Board Certification via AANP or

ANCC MSN

degree from an accredited nursing school Current valid CPR (BLS) certification At least 6 months of recent Nurse Practitioner experience preferred This position offers a pay structure of $80-$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
