Key Responsibilities:

Conduct thorough annual home health risk assessments for Medicare Advantage plan members Evaluate patient's prior and current medical history, medical conditions, and medication use Offer education to patients regarding their medical conditions and medications Identify and document gaps in care, providing clear recommendations for follow-up

Qualifications:

Active Nurse Practitioner license in West Virginia Board Certification through AANP or ANCC Current CPR certification (BLS) Graduation from an accredited MSN program At least 6 months of current experience as a Nurse Practitioner preferred In this role, you will be compensated at $80-$90 per evaluation, plus a mileage stipend and reimbursement for administrative time.
