Responsibilities:

Perform thorough annual or post-discharge home health risk assessments for Medicare Advantage members Review patients medical history, current conditions, and medication usage Educate members about their medical conditions and prescribed medications Identify and communicate gaps in care, offering actionable recommendations Focus exclusively on clinical assessment; no need to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in West Virginia Current BLS (CPR) certification Board certified by either AANP or ANCC At least 6 months of recent NP experience (preferred) Master of Science in Nursing (MSN) from an accredited program
