Key Responsibilities:

Deliver skilled in-home nursing care and interventions Complete comprehensive patient assessments and OASIS documentation with accuracy and timeliness Collaborate closely with physicians and multidisciplinary care teams to implement personalized care plans Provide wound care, medication management, chronic disease monitoring, and patient education Clearly communicate updates and any changes in patient conditions to the clinical team

Qualifications:

Active RN license (required) Minimum one year of recent home health RN experience OASIS documentation experience Strong proficiency with HomeCare HomeBase (preferred) Reliable transportation and a valid drivers license Ability to manage a structured schedule involving daily patient visits
