Job Description
Primary Duties and Responsibilities The Intake Coordinator is responsible for the timely and accurate entry of a variety of data, including patient information, and billing and payroll information. Responds to referrals from a wide range of referral sources. Assists the functions of patient care management through thorough, timely, and accurate data entry and system reporting. Communicates patient needs to the clinical team via entry in to the electronic medical record. Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for care, plan of care initiation, and coordination of care with multiple service providers. Ensures maximum third party reimbursement through direct oversight of insurance verification and authorization processes. Maintains comprehensive working knowledge of Hoag at Home contractual relationships and ensures that patients are admitted according to contract provisions. Education and Experience High School diploma or equivalent required Two (2) years college preferred.
