Director of Nursing/Director of Patient Care Services (DON/DPCS) Graceland Hospice Care Graceland Hospice Care is seeking an experienced, compassionate, and highly organized Director of Patient Care Services (DPCS) to lead our hospice clinical operations and interdisciplinary clinical team. The DPCS is responsible for the overall direction, supervision, quality, compliance, and coordination of patient care services. This leadership position works closely with the Administrator, Medical Director, Clinical Supervisors, RN Case Managers, LVNs, CHHAs/Hospice Aides, Social Workers, Chaplains, and other members of the interdisciplinary team. Key Responsibilities Direct and oversee day-to-day hospice clinical operations and patient care services. Supervise Clinical Supervisors, RN Case Managers, LVNs, Hospice Aides/CHHAs, and other clinical personnel. Ensure appropriate staffing, scheduling, workload distribution, and clinical coverage based on census and patient acuity. Ensure timely admissions, assessments, visits, follow-up, medication coordination, and implementation of patient plans of care. Monitor hospice eligibility, certifications/recertifications, clinical documentation, IDG documentation, and evidence of patient decline. Maintain compliance with CMS Medicare Hospice Conditions of Participation, CDPH requirements, California hospice regulations, and Joint Commission standards . Maintain continuous survey and accreditation readiness. Lead and participate in QAPI , clinical audits, performance improvement projects, corrective action plans, and Plans of Correction. Oversee clinical preparation and responses for ADRs, TPEs, Medicare audits, medical-record requests, CDPH investigations, and accreditation surveys . Review clinical records for completeness, hospice eligibility, terminal prognosis, physician narratives, face-to-face documentation, and regulatory compliance. Investigate patient complaints, medication errors, falls, missed visits, hospitalizations, documentation deficiencies, and other clinical or patient-safety concerns. Participate in recruitment, orientation, training, competency evaluations, performance evaluations, coaching, corrective counseling, and disciplinary recommendations for clinical staff. Promote excellent patient care, accountability, teamwork, timely communication, accurate documentation, and compassionate end-of-life care. Provide clinical leadership for infection prevention, emergency preparedness, medication management, symptom management, and risk management. Maintain effective communication with physicians, pharmacies, DME providers, facilities, contracted providers, and other healthcare partners. Qualifications Current and unrestricted California Registered Nurse (RN) license required. Significant hospice and/or home health clinical leadership experience. Previous experience as a Director of Nursing, DPCS, Clinical Director, Clinical Supervisor, or comparable healthcare leadership position strongly preferred. Strong knowledge of Medicare Hospice Conditions of Participation (42 CFR Part 418) . Experience with CDPH hospice surveys and regulatory compliance . Joint Commission accreditation/survey experience preferred. Experience with ADRs, TPEs, CMS/Medicare audits, QAPI, corrective action plans, and Plans of Correction highly desirable. Strong understanding of hospice eligibility, clinical documentation, certification/recertification, IDG, and plan-of-care requirements. Experience managing a high-volume hospice census and interdisciplinary clinical team preferred. Hospice EMR experience; HospiceMD experience preferred. BSN or higher nursing degree preferred. Excellent leadership, communication, organizational, problem-solving, and staff-management skills.
