Job Description

First Choice Health Centers is seeking a Nurse Practitioner or Physician Assistant for our primary care practice in Manchester, CT. This position is accountable for independently performing a full range of tasks in delivery of direct patient care in an outpatient setting. Why First Choice? We are committed to you! Experience and Training :
Proven experience as a Nurse Practitioner or Physician Assistant establishing care plans and managing patient outcomes A current license to practice as a NP or PA in the state of Connecticut. Appropriate Board Certification A commitment to serving diverse and vulnerable populations with compassion and respect. Excellent communication, interpersonal, and organizational skills. Connecticut Controlled Substance license and DEA license BLS Certification through the American Heart Association or American Red Cross. Pay is based on several factors including but not limited to work experience certifications, etc. Range for this position is $115,000 - $160,000
Standard Job Duties:
Evaluates, assesses patient illness/injuries. Chronic care management Performs physical examinations, and special examinations. Takes medical histories. Develops plans of care. Provides routine and illness care. Orders medications; orders diagnostic tests; and evaluates results. Assumes responsibility for patient follow-up, coordination, and referrals to other health care providers as necessary. Maintains patient medical records. Provides patient counseling/education. Participates in medical student and health professional teaching. Performs related duties as required.
