Admissions Coordinator - Skilled Nursing Facility Full-Time | On-Site | Skilled Nursing & Rehabilitation We are seeking an experienced, organized, and compassionate Admissions Coordinator to join our skilled nursing and rehabilitation team. The Admissions Coordinator plays a key role in coordinating the admissions process from referral through arrival, ensuring that residents, families, referral partners, and internal departments receive timely and accurate communication. This position is ideal for someone who understands the SNF admissions process , is comfortable working with hospitals, physicians, insurance companies, residents and families, and can effectively manage multiple admissions and priorities in a fast-paced healthcare environment. What You'll Do Coordinate the admission process for new residents from referral through arrival. Review and coordinate incoming referrals and ensure required information is obtained. Communicate with hospitals, hospitals' case managers, discharge planners, physicians, residents, and families regarding potential admissions. Obtain and verify insurance information, benefits, authorizations, and other required admission information. Coordinate with clinical and administrative departments to ensure the facility is prepared for each admission. Maintain accurate and timely documentation related to referrals and admissions. Communicate admission details and special requirements to the appropriate departments prior to the resident's arrival. Assist with scheduling and coordinating admission times and transportation as needed. Follow up on pending referrals and maintain communication with referral sources. Help ensure admissions are processed efficiently and accurately while maintaining compliance with facility policies and applicable regulations. Provide a welcoming and professional experience for residents and their families during the admission process. Support census development by maintaining strong relationships with referral sources and providing excellent customer service. Assist with identifying barriers to admission and communicating concerns promptly to the appropriate leadership team. Work closely with the Administrator, Director of Nursing, Social Services, Business Office, and other departments to facilitate successful admissions. What We're Looking For Previous Skilled Nursing Facility (SNF) admissions experience strongly preferred . Healthcare admissions, referral coordination, hospital discharge planning, or healthcare administrative experience preferred. Strong understanding of the SNF admission process. Knowledge of Medicare, Medicaid, managed care, and commercial insurance is a plus. Excellent communication and interpersonal skills. Strong organizational skills and attention to detail. Ability to prioritize multiple referrals and admissions in a fast-paced environment. Professional, friendly, and service-oriented approach. Ability to work collaboratively with clinical and administrative teams. Strong computer and documentation skills. Ability to maintain confidentiality and handle sensitive resident information appropriately. Why Join Our Team?
