Job Description
Job Overview We are seeking a highly skilled and dedicated Physician Assistant or Family Nurse Practitioner to join our dynamic healthcare team. The ideal candidate will possess comprehensive clinical expertise, a passion for patient-centered care, and have experience in primary care. Key Responsibilities Provide comprehensive primary care services including preventive care, acute visits, and chronic disease management Strong knowledge of medical terminology, anatomy, physiology, and pharmacology necessary for effective patient assessment and treatment planning. Diagnose and manage common primary care conditions such as hypertension, diabetes, hyperlipidemia, COPD/asthma, obesity, chronic pain, sleep disorders, thyroid disorders, and mental health conditions, among others Perform and interpret routine diagnostic tests (labs, EKGs, ABIs) Prescribe medications and develop evidence-based treatment plans Document encounters accurately and efficiently within the electronic medical record Educate patients on disease prevention, lifestyle modification, and treatment adherence Maintain continuity of care through longitudinal patient relationships, including appropriate follow-ups and monitoring of treatment response Conduct annual wellness visits, health risk assessments, and age-appropriate preventative screenings Manage transitional care following hospitalizations or emergency department visits Review, interpret, and act on laboratory results, imaging studies, and diagnostic reports in a timely manner Schedule & Practice Environment Full-time outpatient position No inpatient responsibilities No call requirements Supportive physician supervision and collaborative team structure Dedicated clinical and administrative support staff Monday-Friday, 8:00 a.m. - 4:00p.m. Focus on quality, continuity, and patient-centered care—not volume-driven medicine Compensation & Benefits Competitive salary commensurate with experience Health Insurance Paid time off
