Assisting Hands- Brevard is looking for a compassionate and organized Referral and Intake Coordinator to join our growing team in Rockledge (Melbourne), FL.
Responsibilities:
Manage incoming referrals from hospitals, physicians, case managers, and community partners Conduct client intake assessments and gather necessary information to initiate home care services Coordinate with care teams to ensure smooth onboarding and seamless transitions for new clients Maintain accurate and up-to-date client records in care management systems Communicate effectively with clients, families, and referral sources throughout the intake process Track and follow up on pending referrals to maximize conversion and client satisfaction Collaborate with schedulers and care coordinators to match clients with appropriate caregiversRequirements:
Prior experience in home care, healthcare intake, case management, or a related field preferred Strong organizational skills with the ability to manage multiple referrals simultaneously Excellent verbal and written communication skills Compassionate, client-focused demeanor with a commitment to quality care Proficiency with care management software or CRM systems a plus Knowledge of home care, Medicaid, or insurance authorization processes is a bonus High school diploma or equivalent required; associate's or bachelor's degree preferredAbout Us:
