In this role, you deliver hands-on therapy and coordinate individualized treatment plans for patients recovering from major illness or injury. Working within an interdisciplinary team, you collaborate with physicians, nurses, and therapists to track progress, educate patients and families, and support safe transitions home. Responsibilities Deliver direct, evidence-based rehabilitation care to an assigned caseload of patients. Complete comprehensive assessments and develop individualized treatment plans. Collaborate with an interdisciplinary team to coordinate and adjust care plans. Document assessments, treatments, and progress accurately in the EMR. Educate patients and families on recovery, safety, and home care strategies. Monitor clinical outcomes and participate in quality improvement initiatives. Support safe discharge planning and transitions across care settings. Participate in ongoing professional development, training, and specialty education.
