A Registrar or Patient Service Representative provides direct assistance to patients at a health care or long term care facility. Answers questions and helps resolve complaints; provides liaison with health care providers; may assist with coordinating appointments or addressing billing or insurance issues.
Intake Coordinator - Behavioral Health - (Bachelors in RN Nursing, Psychology, Social Work, Chemical Dependency or related required) Horizon health is seeking an Intake Coordinator to join our team at Reid Hospital & Health Care Services in Richmond, IN. The Intake assessor, in conjunction with the Behavioral Health Program Director may coordinate the referral development and pre-admission patient assessment process and provide clinical assessments for potential clients.
Responsibilities:
Responds to intake calls and completes admission paperwork. Assessments will be completed by RN, LPC or LMSW Facilitates groups, family and individual sessions and completing all necessary documentation Schedules Unit assessments, meetings and appointments as appropriate Utilizes assessment tools including, but not limited to Geriatric Depression Scale, Zung, SLUMS, alcohol screening tool(s) under authorization/supervision of RN, LPC or LMSW Communicates with referral sources including, family members, legal system, psychiatrists, primary care physicians, and other treatment facilities. Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships Participates as an active member of the interdisciplinary team Maintains compliance with state and federal confidentiality laws as well as upholding our Horizon's confidentiality policies and procedures. Identifies areas needing improvement and utilizes the facility performance improvement process. Actively participates in department processes as requested Enters required data into HorizonPlus system and generates monthly reports to track and trend referral, admission and discharge data. Assists in submission of reports and requests for documents from various regulatory agencies; re: PA Dept. of Health, CAP and Joint Commission Prepares and disseminates monthly reports and completes required follow up on activities on behalf of the Director. Prepares and submits all operational tasks/documentation (census report, PIORS) as required by the corporate offices Other duties as assigned
Requirements:
