Overview Join our compassionate healthcare team as a Hospice Intake Coordinator, where your organizational skills and empathetic communication will play a vital role in delivering quality end-of-life care. In this dynamic position, you will serve as the first point of contact for referral sources, coordinating intake processes with efficiency and sensitivity. Your expertise will ensure smooth transitions into hospice services while maintaining compliance with health regulations and confidentiality standards. This role offers an opportunity to make a meaningful difference in patients' lives through dedicated support and professional excellence. Duties Function as the primary contact for new hospice patient referrals, gathering essential medical and personal information to facilitate intake. Coordinate with healthcare providers, insurance companies, and family members to verify eligibility and streamline admission procedures. Utilize software and Electronic Health Records (EHR) systems "Homecare Homebase" to accurately document patient details, history, and care plans. Communicate clearly and compassionately via phone and written correspondence with patients, families, and medical teams to answer questions and provide guidance. Ensure all documentation complies with health regulation policies, maintaining strict confidentiality of patient information at all times. Schedule assessments, follow-up appointments, and coordinate necessary services to support timely hospice admission. Perform medical administrative tasks such as reception, data entry, clerical, managing appointment calendars, and maintaining organized records. Requirements Proficiency in Homecare Homebase Electronic Health Records (EHR) management systems a must. Proven experience working in a medical office or healthcare setting, preferably with hospice or palliative care services. Proficiency in Microsoft Office applications including Word, Excel, and Outlook for documentation and communication purposes. Strong knowledge of health regulation policies related to patient privacy (such as HIPAA) and medical recordkeeping standards. Familiarity with Medical terminology, Medical administrative support functions. Excellent customer service skills with the ability to communicate effectively over the phone and in person with diverse populations. Knowledge of health insurance processes and requirements to assist patients with coverage questions. Ability to manage sensitive information discreetly while maintaining high levels of accuracy through typing and data entry skills. Office experience that demonstrates organizational skills, attention to detail, and the ability to multitask efficiently. Ability to communicate with multiple departments effectively.
