Job Description
The Intake Coordinator serves as the first point of contact for veterans, family members, walk-ins, and community partner referrals at the Bastion Veteran Wellness Center. This role is responsible for conducting initial needs assessments, coordinating referrals, maintaining accurate client records, and connecting individuals with appropriate services and resources. Summary/Objective:
The Intake Coordinator serves as the first point of contact for veterans, family members, walk-ins, and community partner referrals at the Bastion Veteran Wellness Center. This role is responsible for conducting initial needs assessments, coordinating referrals, maintaining accurate client records, and connecting individuals with appropriate services and resources.Essential Functions & Responsibilities:
Collaborate with the Social Worker team, Building Manager, and Clinical Director to consistently execute intake protocol, including referrals to partner organizations. Conduct initial assessments to gather basic information for walk-ins (Veterans, family members, all new community contacts). Evaluate clients' needs and determine eligibility for services. Organize and input data into current systems: Salesforce. Advocate on behalf of and along with clients/veterans to access the services needed. Support clients in independently completing paperwork and applications. Follow up with clients/veterans/caregivers regarding recent referrals. Identify points of contact and maintain relationships with these veterans' resources, vet contact information and eligibility for services annually. Stay up to date about local and regional community resources and services and update Bastion resource. database with any changes to availability or nature of resources and providers.Education, Experience, & Qualifications:
Bachelor's degree in Social Work, Psychology, Human Services, or a related field (required or strongly preferred). Experience in social services, case management, intake coordination, or a related role (preferred). Knowledge of veteran services, community resources, and support systems (preferred). Strong communication, organizational, and interpersonal skills. Empathetic, culturally responsive, and nonjudgmental approach to client care. Ability to manage multiple tasks while maintaining attention to detail and professionalism.Work Environment & Demands:
