Job Description

The New Client Intake Coordinator serves as one of the first points of contact for prospective clients and their families at North Star Care at Home. This position is responsible for guiding families through the intake and onboarding process with warmth, compassion, professionalism, and attention to detail.
Key Responsibilities:
Serve as a primary point of contact for new client inquiries received by phone, email, website, referral partners, and other sources. Conduct initial intake conversations with prospective clients and/or family members and ensure client records are accurate, organized, and complete. Schedule and take lead on initial consultations and/or assessments with prospective clients and family members. Prepare and distribute required service agreements, intake paperwork, policies, acknowledgments, and other documentation. Ensure required paperwork is completed and returned prior to the start of services whenever applicable. Obtain necessary client contacts, emergency information, physician information, and other required documentation.
Administrative/Office Support Responsibilities:
Maintain complete and organized client intake records. Enter and update client information accurately in agency software. Monitor outstanding intake paperwork and follow up as needed. Maintain intake checklists to ensure all required steps are completed before services begin. Assist with preparing client folders, care information, and onboarding materials. Maintain confidentiality and comply with HIPAA and agency privacy requirements. Assist with general administrative responsibilities related to new clients and client services. Participate in office meetings and communicate updates regarding pending and newly admitted clients. Qualifications Preferred Experience Previous experience in home care, healthcare, senior services, medical administration, client intake, scheduling, customer service, or a related field preferred. Experience communicating with older adults and families is strongly preferred. -Familiarity with home care services, CNAs, HHAs, companions, dementia care, hospice support, or private- duty care is beneficial. Experience with scheduling or client management software is a plus. Pay is negotiable based upon experience
