This role serves as the front line of referral intake and patient onboarding, combining clinical review of incoming referrals with the coordination that keeps referral sources, clinical staff, scheduling, and operations moving together. The ideal candidate is a licensed nurse with a home health background who is meticulous, professional, able to multitask in a fast-paced healthcare environment, and committed to delivering excellent service to patients, families, referral sources, and staff. Key Responsibilities Receive, process, and track incoming home health referrals from hospitals, physician offices, case managers, rehab facilities, and community sources Perform clinical review of referral documentation for completeness, accuracy, and eligibility — including homebound status, medical necessity, and appropriateness for home health services Apply clinical judgment when matching referrals to the appropriate discipline and level of care Communicate promptly with referral sources to obtain missing documentation or clarify physician orders Coordinate timely assignment of referrals to nursing and clinical staff based on geography, discipline, acuity, availability, and urgency Work closely with the scheduling team to ensure timely Start of Care visits and admissions Escalate clinically complex or borderline referrals to the Director of Nursing / Clinical Manager Maintain daily referral logs and intake tracking systems Follow up on pending referrals, insurance authorizations, physician orders, and outstanding documentation Communicate referral status updates to leadership and referral partners Verify demographic, insurance, and payer information Ensure compliance with agency intake procedures, privacy standards, and regulatory requirements Support census growth by ensuring referrals are processed efficiently and professionally Assist with administrative projects and workflow improvement initiatives as needed Qualifications Required Active Massachusetts LPN or RN license in good standing Strong administrative and organizational skills Exceptional attention to detail and accuracy Ability to prioritize multiple tasks and meet deadlines Strong phone communication and customer service skills Professional demeanor with patients, families, and referral sources Proficiency in EMR systems, Microsoft Office, email, and data entry Ability to work independently while collaborating with a team Preferred Home health experience Previous experience in healthcare intake, admissions, authorization, scheduling, or medical office administration Knowledge of home health referral processes, Medicare eligibility criteria, insurance verification, and prior authorization Familiarity with OASIS and Start of Care documentation requirements

$60,000.00 - $65,000.00 per year
