Summary:
The Registered Nurse - Patient Navigator - MHMG MMCP role guides patients through the care continuum, establishing contact with recurrent and newly diagnosed cancer patients and/or family members, or caregivers. The Patient Navigator will understand the clinical pathways established by the Oncology Program and will direct the patients to health care services within the organization, at outside facilities, and within the community for timely diagnosis, treatment and survivorship. Required Minimum Knowledge, Skills, and Abilities (KSAs)
Education:
Graduate from an accredited School of Nursing required. Bachelor's Degree in Nursing required or must complete BSN within three years of start date.
License/Certifications:
Current applicable state(s) license as a Registered Professional Nurse required. Oncology certification or defined process for achieving certification required.
Experience:
Experience working with oncology patients and families preferred. Experience in community outreach preferred.
