We are seeking a compassionate, organized, and motivated Social Services & Admissions Coordinator to join our skilled nursing facility team. This position plays a vital role in supporting residents and families throughout the admission process, coordinating transitions of care, assisting with discharge planning, and helping residents achieve the highest possible quality of life during their stay. The ideal candidate will have strong communication skills, a customer-service mindset, and the ability to build relationships with hospitals, referral sources, residents, families, and interdisciplinary team members. Essential ResponsibilitiesAdmissions & Census Development Coordinate and manage the facility admission process from referral through admission. Review referrals and collaborate with nursing leadership to determine clinical appropriateness for admission. Communicate promptly with hospitals, case managers, discharge planners, physicians, and referral sources regarding admission decisions. Facilitate resident and family tours and provide information regarding services offered by the facility. Maintain accurate admission records and ensure all required documentation is completed prior to admission. Assist with census development initiatives and maintain positive relationships with referral sources and community partners. Participate in marketing and outreach efforts to support facility occupancy goals. Social Services Conduct psychosocial assessments and assist residents in identifying social, emotional, and psychosocial needs. Support residents and families with adjustment to placement, transitions of care, and discharge planning. Advocate for resident rights, dignity, and quality of life. Assist residents and families with advance directives, community resources, and support services. Participate in care plan meetings and interdisciplinary team meetings. Address resident and family concerns and work collaboratively to achieve positive resolutions. Assist with discharge planning to ensure safe and successful transitions to the next level of care. Regulatory Compliance Maintain compliance with federal, state, and facility regulations related to social services and admissions. Ensure documentation is completed timely and accurately within the electronic medical record. Support survey readiness efforts and participate in quality improvement activities. Maintain confidentiality and comply with HIPAA requirements. Qualifications Bachelor's degree in Social Work, Human Services, Psychology, Healthcare Administration, or related field preferred. Experience in a skilled nursing facility, rehabilitation center, long-term care, hospital case management, or healthcare admissions strongly preferred. Knowledge of Medicare, Medicaid, managed care, and post-acute care services preferred. Strong organizational, communication, and relationship-building skills. Ability to manage multiple priorities in a fast-paced healthcare environment. Proficiency with electronic medical records and Microsoft Office applications. Why Join Our Team?
