$115,000-$135,000 + Comprehensive Benefits Position Overview A leading healthcare organization is seeking a Family Nurse Practitioner (FNP) to join its outpatient primary care team. This is an excellent opportunity to provide comprehensive, patient-centered care while building long-term relationships with individuals and families in a collaborative clinical environment. Responsibilities Provide comprehensive primary care services for patients of all ages. Diagnose, treat, and manage acute and chronic medical conditions. Perform preventive care, annual wellness visits, health screenings, and patient education. Develop personalized treatment plans and coordinate ongoing patient care. Collaborate with physicians, nurses, and interdisciplinary healthcare teams. Maintain accurate and timely documentation using Electronic Medical Records (EMR). Promote evidence-based practice and participate in quality improvement initiatives. Qualifications Licensed Family Nurse Practitioner (FNP) . Active RN license or eligibility for RN licensure in New York. Experience in Primary Care, Family Medicine, Internal Medicine, or Community Health preferred. Strong clinical assessment and decision-making skills. Excellent communication and patient education abilities. Passion for providing compassionate, relationship-based care.
