Job Description

Team Select Homecare is looking for Licensed Practical Nurses to join our Team Select family. Our nurses will document and provide skilled nursing care in a 1:1 setting to our patients in their home. Tuition Reimbursement Locations:
Drexel Hill, Upper Darby, Media, Havertown, Chester We work with pediatric and adult patients! Assist the patient in learning appropriate self-care techniques and assure that the patient/family is involved in implementing the plan of care Responsibilities Implement the nursing care plan for each patient. Initiate preventive and rehabilitative nursing procedures. Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home. Maintain and complete records of observations, treatments and care of patient. Help update the patient plan of care. Administer medications and treatments as ordered. Demonstrate competent performance of technical skills according to established procedures before performing new skills. #
