$18-$25 per hour We are looking for a highly organized, compassionate, and dependable professional with experience in healthcare intake, home care, admissions, care coordination, or a related healthcare role. Key Responsibilities Serve as a primary contact for new clients, families, and referral sources Conduct intake calls and gather client information Explain services and guide clients through the intake process Coordinate assessments, appointments, and admissions Collect and track required documentation Follow up on pending referrals and information Maintain accurate client records Coordinate with internal teams for a smooth transition to services Handle calls, emails, and SMS professionally Address client and family questions and concerns Maintain confidentiality and follow HIPAA requirements Qualifications Previous intake or healthcare coordination experience
