The Participant Intake Navigator conducts referral eligibility screenings, assists with and processes waiver referrals, manages initial phone contacts, and develops strong working relationships with Service Coordinator entities. This role is also responsible for coordinating field assessors to conduct home evaluations, monitoring subcontractor bid information through Buildertrend software, and managing pre-submission workflows and database documentation. The Participant Intake Navigator plays an essential role in ensuring referrals move efficiently through the intake process and that all required information is accurate and complete.

EDUCATION/EXPERIENCE

HighSchoolDiplomaRequired.

ExperienceinCustomerServiceorCallCenterpreferred. Previousexperiencewithindividualswithdisabilitiesaplus.

$17-20 per hour

Monday-Friday between 8-5pm, opportunity to work remotely
