The Admission Coordinator is responsible for managing the referral and admission process for the nursing facility. This position serves as the primary liaison between hospitals, families, physicians, and the facility to coordinate timely and appropriate admissions. The coordinator ensures all required documentation, insurance verification, and clinical information are obtained prior to admission.
Responsibilities:
Coordinate and manage all incoming referrals and admissions to the facility Communicate with hospitals, physicians, discharge planners, and families regarding placement Review referral information to determine admission eligibility and appropriateness Verify insurance coverage and benefits prior to admission Complete admission paperwork and ensure required documentation is obtained Conduct facility tours for potential residents and families Work closely with nursing, social services, and administration to coordinate admissions Track referrals and maintain accurate admission records Represent the facility professionally in the community and healthcare settings
Qualifications:
Healthcare experience required; nursing home or hospital experience preferred Knowledge of Medicare, Medicaid, and managed care insurance Strong communication and relationship-building skills Excellent organizational and follow-through abilities Ability to work with hospitals, case managers, and families in a fast-paced environment Proficiency with computers and electronic documentation systems
