Job Description

Job Overview We are seeking a dedicated and compassionate Nurse - Intake Coordinator to join our healthcare team. This role is crucial in ensuring that new patients receive a thorough assessment and seamless transition into our care facility. The ideal candidate will possess a strong clinical background, excellent communication skills, and a commitment to providing high-quality patient care. Experience in various nursing specialties is highly valued, as you will be working with diverse patient populations. The position is Part Time working every other Friday, Saturday, and Sunday 8:00AM- 8:30PM Onsite (3 12's) Responsibilities Conduct comprehensive patient assessments during the admissions process, including medical history and physical. Collaborate with interdisciplinary teams to develop individualized care plans tailored to each patient's needs. Ensuring adherence to safety protocols and best practices. Provide education to patients and families regarding treatment plans, medication administration, and health management strategies. Maintain compliance with HIPAA regulations while handling sensitive patient information. Participate in discharge planning and follow-up care coordination for patients transitioning out of the facility. Engage in ongoing professional development to stay current with best practices in nursing care. Qualifications Current Registered Nurse (RN) licensure in the state of practice. Experience in acute care settings, psychiatric hospital, or long-term care preferred. Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), and medical documentation practices. Strong assessment skills with the ability to recognize changes in patient conditions promptly. Previous experience working with diverse populations, including geriatrics. Knowledge of pain management techniques, infection control practices, and behavioral health considerations is beneficial. Ability to work collaboratively within a team-oriented environment while demonstrating strong interpersonal skills.
