Job Description We are seeking an exceptional and compassionate Nurse Practitioner or Physician Assistant to join our dedicated Palliative Care team. This is a meaningful opportunity to make a lasting difference in the lives of patients and families during some of their most vulnerable moments. In this role, you will provide holistic and supportive care for patients with advanced illnesses
  • primarily in oncology, neurology (stroke, dementia, etc.
), pulmonary (COPD, respiratory failure, etc.), and gastrointestinal diseases. You'll be part of a collaborative interdisciplinary team focused on improving quality of life and managing symptoms.   • 5:00pm Monday
  • Friday 6
  • 10 patients per day, a mix of new admits and follow up patients 2 Physicians and 4 APPs Collaborate with families, patients, social workers, and care teams across the system Call
  • 1:4 one weekend a month and one weekday a week
  • calls are usually taken from home, will require coming to see the patient occasionally.

Qualifications Required Education:
Graduate of an accredited Nursing Program, and Master's of Science Degree in
Nursing Preferred Experience:
Minimum of two (2+) years of progressive experience in area of specialization
Required Certifications and Licensures:
Current licensure by the Virginia State Board of Nursing as a Nurse Practitioner with prescriptive authority. Maintains BLS and ACLS Certifications. Hold a current, active license as a registered nurse in Virginia or hold a current multistate licensure privilege as a registered nurse. Nationally recognized certification in field of specialization
