Colchester, Vermont, United States Job Description Overview:
This position acts as the initial point of contact for individuals, families and referring agencies for people who are interested in receiving services at CCS. The primary purpose of this role is to gather, process and manage client information and integrate it into CCS' systems, caseloads and services. The scope of the Intake Coordinators role involves collaborating with community partners, such as other DS agencies, case management organizations, and schools, and building trusting relationships with individuals and families.
Requirements:
Bachelors' Degree in human services-related field (or equivalent experience), plus two years experience in developmental services. Valid driver's license, a good driving record, and the ability to provide own transportation.
Areas of Accountability:
A) Intake  Receive and respond to new client referrals  Schedule and conduct meetings with families, individuals and care teams  Attend school meetings, such as IEP meetings, as they arise  Outreach and informational sessions at schools or local agencies  Educate families on the developmental services eligibility process and available resources  Collect, review and maintain individual records  Work closely with individuals, families, case management organization and referring agency or school to determine individual support needs  Work closely with internal team to determine if CCS can meet the needs of the individual and if team has capacity.  Ensure all required documentation is received  Open individual in Electronic Health Record system  Review funding with Associate Director  Work with individual and Workforce Development Senior Manager to determine schedule and staffing requirements.  Connect with appropriate personnel regarding posting for a position if required. B)
Service Coordination:
 Follow individual through their transition to CCS to ensure a smooth transition of services  Attend and contribute to annual service plan meeting.  Write other support plans as needed with support from team.  Develop, coordinate and execute intervention and behavioral support strategies to support client and community safety. Evaluate and modify these strategies through ongoing observation and review of team input  Coordinate physical environment inspections or modifications to ensure accessibility or safety for client and community. Homeward Program Manager Job Description  Develop, coordinate and execute intervention strategies for clients in crisis.  Development of different long or short term living arrangements and recruitment of home providers as necessary.  Participate in the on-call rotation to provide evening and weekend supports.  Advise or respond to emergent needs on the weekends and evenings as available.  Assist the client in identifying, developing and/or maintaining natural supports. C) Other /
Agency/Client Support:
 Responsible for hiring, training, and supervising a varying number of Shared Living Providers in a manner that supports the contractors' development and success within the program. Monitor and manage residential contracts. Responsibilities include regular and consistent oversight to ensure that all terms of the contract and regulations are adhered to and that quality supports are delivered. D)
Documentation:
Document service provision and other required information to provide quality service and ensure compliance with regulatory standards and funding allocation by:  Reviewing an annual service plan, tracking documentation and related communication, personal, and safety plans.  Monthly team meeting notes and home visit form  Home visit forms.  Incident reports.
