LPN Intake Coordinator Columbia River Home Health | Kennewick, WA Are you an organized, customer-focused LPN looking to make an impact behind the scenes of patient care? Columbia River Home Health is seeking an LPN Intake Coordinator to manage referrals, coordinate admissions, verify insurance, and serve as a key liaison between patients, families, referral partners, and our clinical team. Competitive pay $32-39 an hour, DOE Monday-Friday schedule Supportive team environment Make a direct impact on patient access to care Growth opportunities within a respected home health organization What You'll Do Manage incoming referrals and coordinate admissions Review referrals for clinical appropriateness Communicate with patients, families, physicians, hospitals, and referral sources Verify insurance benefits and authorizations Coordinate seamless transitions of care Maintain accurate documentation and patient records Build and maintain strong relationships with referral partners Qualifications Current Washington State LPN license Healthcare experience required; home health, hospice, intake, referrals, or care coordination experience preferred Strong communication, organization, and customer service skills Ability to multitask and work independently in a fast-paced environment EMR and computer proficiency
