In this vital role, you will deliver personalized speech and language therapy services directly in patients' homes, fostering independence and enhancing quality of life. Your expertise will support diverse populations, including pediatric and geriatric patients, through comprehensive assessments, tailored therapy plans, and collaborative care. Duties Conduct thorough speech and language evaluations for pediatric and adult patients, including early intervention assessments for children. Develop, implement, and monitor individualized speech-language therapy plans that address specific communication and swallowing needs. Provide direct therapy sessions using evidence-based techniques across various settings, including telehealth platforms when appropriate. Collaborate closely with families, caregivers, teachers, and other healthcare professionals to ensure cohesive treatment approaches and optimal patient outcomes. Educate parents and caregivers on strategies to support communication development or rehabilitation at home. Maintain accurate documentation of patient progress, assessments, treatment plans, and session notes within Electronic Medical Records (EMR) or Electronic Health Records (EHR) systems. Stay current with medical terminology, physiology, anatomy knowledge, and best practices in speech-language pathology to deliver high-quality care. Skills Extensive experience working with children in pediatrics, early intervention programs, or school settings. Strong background in speech therapy and language therapy across diverse populations with speech impairments or disabilities. Proven ability to implement speech-language therapy plans effectively for both pediatric and geriatric patients. Excellent patient assessment skills with a focus on comprehensive speech and language evaluations. Proficiency in telehealth therapy delivery and familiarity with telehealth platforms to expand access to care. Knowledge of physiology and anatomy related to speech production, swallowing function, and communication disorders. Experience working collaboratively within multidisciplinary teams to coordinate patient care strategies. Ability to communicate complex medical terminology clearly to patients and families. Familiarity with EMR/EHR systems for documentation and reporting purposes.
