Overview We are seeking a skilled and compassionate Speech Language Pathologist to join our clinical team. This position plays a critical role in delivering medically prescribed, goal-oriented therapy to patients receiving home health services. The SLP assesses and treats speech, language, cognitive, and swallowing disorders to help patients achieve greater independence and improved quality of life. Key Responsibilities- Conduct comprehensive assessments and develop individualized Plans of Care in collaboration with physicians and interdisciplinary team members
- Provide evidence-based therapeutic interventions to improve speech, language, cognition, and swallowing functions
- Continuously evaluate patient progress and adjust care plans as needed
- Document care accurately and in a timely manner, ensuring compliance with agency and regulatory standards
- Foster strong communication with patients, families, physicians, and care teams to ensure continuity of care
- Participate in interdisciplinary case conferences and quality initiatives
- Adhere to agency policies, accreditation standards, and state and federal regulations Qualifications
- Master's degree in Speech-Language Pathology from an accredited institution
- Active SLP license in the state of practice in good standing
- Minimum one year of experience in a home health or medical setting preferred
- Knowledge of state practice acts, ACHC standards, and home health documentation requirements
- Current CPR certification and valid driver's license with reliable transportation
- Strong communication, clinical judgment, and documentation skills Work Environment This position involves regular travel to patient homes and care facilities.
