Job Description

Speech Therapist (SLP)
Position Summary:
The Speech Therapist (SLP) evaluates, diagnoses, and treats communication, cognitive, and swallowing disorders. This role focuses on helping individuals improve their speech, language, voice, cognition, and safe swallowing abilities to maximize independence and quality of life.
Key Responsibilities:
Evaluate residents/patients for communication, cognitive, and swallowing needs Develop and implement individualized treatment plans based on assessments and physician orders Provide therapy services to improve speech, language, voice, fluency, memory, problem-solving, and swallowing safety Document evaluations, progress notes, and discharge summaries in compliance with professional, state, and federal standards Educate residents, families, and staff on communication strategies, swallowing precautions, and home programs Collaborate with physicians, nursing, and other therapy disciplines to ensure coordinated care Participate in interdisciplinary care plan meetings and quality improvement programs Maintain professional licensure and adhere to ethical practice standards
Qualifications:
Current and active Speech-Language Pathologist (SLP) license in Minnesota (or eligibility) Master's degree in Speech-Language Pathology from an accredited program Certificate of Clinical Competence (CCC-SLP) from ASHA preferred Strong assessment, treatment, and documentation skills Excellent communication and interpersonal skills Experience in skilled nursing, rehabilitation, or long-term care preferred
