$50.98 - $61.40 per hour Job description: $65 per point About Moorestown Visiting Nurse Association (Moorestown VNA) For generations, Moorestown Visiting Nurse Association (Moorestown VNA) has been dedicated to delivering compassionate, high-quality home health care throughout South Jersey. Our interdisciplinary team is committed to helping patients recover, improve their independence, and maintain the highest possible quality of life in the comfort of their own homes. We are currently seeking a licensed Speech-Language Pathologist (SLP) to join our Home Health team serving Burlington County, NJ . Position Summary As a Home Health Speech-Language Pathologist, you will evaluate and treat patients experiencing speech, language, cognitive, communication, and swallowing disorders. Working collaboratively with physicians, nurses, therapists, home health aides, patients, and caregivers, you will develop individualized treatment plans that promote independence and improve patient outcomes. This position offers the opportunity to build meaningful relationships with patients while enjoying the flexibility and autonomy of home-based care. Responsibilities Evaluate patients with speech, language, cognitive, communication, and swallowing disorders. Develop individualized treatment plans in collaboration with physicians and the interdisciplinary care team. Provide skilled speech-language therapy services in patients' homes. Educate patients and caregivers on communication techniques, swallowing safety, and home exercise programs. Monitor patient progress and modify treatment plans as appropriate. Coordinate care with physicians, nurses, physical therapists, occupational therapists, social workers, and home health aides. Communicate changes in patient condition and obtain physician orders when necessary. Complete accurate and timely clinical documentation, including evaluations, treatment notes, and discharge planning. Prepare patients and caregivers for continued success following discharge from therapy services. Follow all agency policies, infection control procedures, patient safety standards, and regulatory requirements. Qualifications Current New Jersey Speech-Language Pathologist (SLP) License CPR Certification Master's Degree in Speech-Language Pathology Experience treating adults with neurological, communication, or swallowing disorders preferred Home health experience preferred but not required Strong communication and patient education skills Ability to work independently and manage a flexible schedule Reliable transportation and willingness to travel throughout Burlington County Schedule Full-Time and Per Diem opportunities available Flexible scheduling Occasional weekend and holiday rotation as needed Why Join Moorestown VNA?
