The Phoenix Center is a 138 bed Skilled Nursing Facility located in the historic Valley Forge area in Phoenixville, PA. Job Description The Speech Language Pathologist is responsible for providing rehabilitative therapy to patients upon referral for communication, cognitive and/or swallowing disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services. This position reports to the Director of Rehabilitation Services.
