Job Description

Overview We are seeking a dedicated and highly skilled Speech-Language Pathologist (SLP) to provide comprehensive speech and language therapy services within a home health setting. The ideal candidate will possess a strong background in geriatrics and speech therapy, with a passion for improving communication abilities across diverse patient populations. This role offers an opportunity to work closely with patients of all ages, including children with developmental delays and adults with speech impairments, delivering personalized care that enhances quality of life. Responsibilities Conduct thorough speech and language assessments to identify communication disorders in patients across various age groups. Develop and implement individualized speech-language therapy plans tailored to each patient's specific needs and goals. Deliver evidence-based therapy interventions utilizing techniques suitable for pediatric populations, early intervention, and adult patients with speech or language impairments. Collaborate effectively with families, caregivers, and multidisciplinary teams to ensure cohesive treatment strategies and optimal patient outcomes. Utilize Electronic Medical Records (EMR) or Electronic Health Records (EHR) systems for documentation, progress notes, and treatment planning. Educate patients and their families on communication strategies, exercises, and techniques to support ongoing progress outside of therapy sessions. Adapt therapy methods for telehealth delivery when necessary, ensuring continuity of care through virtual platforms. Maintain up-to-date knowledge of physiology, anatomy, and medical terminology relevant to speech-language pathology to inform assessment and intervention strategies. Qualifications Master's degree in Speech-Language Pathology from an accredited program; licensure or certification as required by state or governing bodies. Proven experience working with children in pediatrics, early intervention programs, or school settings is highly desirable. Demonstrated expertise in speech therapy techniques for individuals with speech impairments, language delays, or disabilities. Familiarity with comprehensive speech and language evaluations and the development of tailored treatment plans. Experience working with geriatric patients or individuals with disabilities is advantageous. Proficiency in EMR/EHR systems for documentation and patient management. Strong understanding of physiology, anatomy related to speech production, and medical terminology pertinent to the field. Telehealth experience is preferred; ability to adapt therapy approaches for virtual delivery is essential.
