Primary Duties & Responsibilities:

Conduct in-home evaluations of speech, language, cognitive-communication, and swallowing function Develop and implement individualized treatment plans based on patient goals and physician orders Provide therapeutic interventions for conditions such as aphasia, dysarthria, apraxia, and dysphagia Educate patients and caregivers on communication strategies and safe swallowing techniques Document all services and progress notes in the EHR system accurately and timely Coordinate with nursing, physical/occupational therapists, and other caregivers to optimize patient outcomes Monitor and reassess treatment plans to ensure measurable improvement Maintain HIPAA compliance and professional standards at all times To Interested applicants: please send resume to our email : assuredbestcare@gmail.com Apply Today or you can contact us at: 817-902-6500
