Job Description
Life Care Centers is seeking a dedicated Speech Language Pathologist to join our Healthcare & Medical Services team. In this role, you will assess, diagnose, and treat speech, language, cognitive-communication, and swallowing disorders for senior residents in a skilled nursing and rehabilitation setting. You will collaborate closely with an interdisciplinary care team to develop individualized treatment plans, document progress, and educate patients and families. Responsibilities Perform comprehensive speech, language, cognitive-communication, and swallowing evaluations for senior residents Develop and implement individualized, evidence-based treatment plans targeting functional communication and safe swallowing Provide direct individual and group therapy services, adjusting interventions based on resident progress and goals Document evaluations, treatment sessions, and progress notes accurately and timely in the electronic medical record Educate residents, families, and caregivers on communication and swallowing strategies, safety, and home programs Collaborate with an interdisciplinary team to coordinate care, establish goals, and participate in care conferences and discharge planning Monitor, record, and report changes in resident status, recommending modifications to the plan of care as needed Ensure compliance with state and federal regulations, professional standards, and facility policies related to speech-language pathology services Contribute to a patient-first, compassionate environment that promotes dignity, respect, and quality outcomes for all residents Participate in ongoing professional development, training, and quality improvement initiatives within the facility Required Skills Speech and language assessment Treatment planning Swallowing (dysphagia) evaluation and treatment Geriatric rehabilitation Electronic medical record (EMR) documentation Interdisciplinary care coordination Patient and family education AAC (augmentative and alternative communication) strategies Cognitive-communication therapy Goal setting and outcomes tracking
