Home Health Nurse

Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and health history reviews, to develop personalized care plans. Administer medications, including injections and IV infusions, following strict protocols and ensuring patient safety. Manage complex care needs such as wound care, catheterization, tube feeding, and ventilator management in home settings. Provide education on disease management, medication adherence, and lifestyle modifications tailored to individual needs. Coordinate with physicians, case managers, and family members to ensure seamless discharge planning and ongoing care. Document all patient interactions accurately using electronic health record (EHR) systems like Epic or Cerner while adhering to HIPAA regulations. Monitor patients' progress through regular evaluations, adjusting care plans as necessary to optimize health outcomes. Skills Extensive experience with Epic EMR systems or similar EHR platforms for efficient documentation. Critical care background with experience in ICU, emergency medicine, or Level I Trauma Centers preferred. Proficiency in managing diverse clinical scenarios such as Alzheimer's care, dementia management, hospice care, and memory care. Strong knowledge of medical terminology, ICD coding (ICD-9/10), and diagnostic evaluation techniques. Skilled in performing patient assessments, vital signs measurement, specimen collection, phlebotomy, and medication administration. Ability to work with individuals with disabilities or developmental disabilities across various settings including assisted living and long-term care facilities. Familiarity with infection control protocols, aseptic techniques, sterile processing, and advanced procedures like spinal taps or cardiac catheterization when applicable. Excellent communication skills for triage calls, patient education, and interdisciplinary collaboration. Knowledge of physiology and anatomy to support accurate clinical decision-making in diverse environments such as outpatient clinics or home & community care settings.
