Travel Contract Assignment Overview We are seeking an experienced Home Health Physical Therapist for a 13-week travel assignment serving Seaside, Oregon and surrounding communities. This role is ideal for a therapist who is comfortable working independently in a home health setting while delivering high-quality patient-centered care.

Seaside, Oregon and surrounding areas Patient & Productivity Information Home Health setting with 1:1 patient care Average of up to 6 patient visits per day Productivity expectation: 30 points standard System designed to minimize travel time between patient visits Job Responsibilities Evaluate and treat patients in their homes according to physician care plans Develop individualized treatment plans focused on mobility, strength, balance, and functional independence Complete OASIS documentation accurately and timely Educate patients and caregivers on therapy goals, exercises, and safety techniques Coordinate care with interdisciplinary healthcare teams Maintain compliance with home health regulations and documentation standards Requirements Minimum 1 year of Skilled Home Health experience required First-time travelers accepted BCLS certification required OASIS experience required Pending license accepted Must have reliable personal transportation For more details reach at sthakur@navitashealth.com or Call / Text at 732 791 4807•EXT 4807 .
