What You'll Do:

Provide hospice nursing visits across homes, assisted living, and memory care settings. Deliver pain and symptom management, wound care, and skilled nursing assessments. Support patients and families through end-of-life transitions with compassion and education. Communicate confidently about care plans and hospice expectations. Work closely with an interdisciplinary team to provide aligned patient care. Maintain documentation supporting continuity and quality outcomes. Participate in an on-call rotation supporting patient care and continuity needs.

What We're Looking For:

Active Minnesota Registered Nurse license. Experience in hospice, ICU, oncology, palliative care, or end-of-life nursing preferred. Strong emotional intelligence and ability to support patients and families. Comfortable working independently in a field-based role. Reliable transportation and ability to travel throughout Southern & Southwest Minnesota service areas.
