What You'll Do:

Perform in-depth in-home evaluations focused on function, safety, and independence. Design customized treatment plans based on real-life environments. Address ADLs, mobility, cognitive function, and home safety barriers. Recommend and implement adaptive equipment and environmental modifications. Educate patients and caregivers to drive lasting outcomes. Document and collaborate with an interdisciplinary team.

What You Bring:

Active Minnesota OTR/L license. Strong clinical judgment and problem-solving ability. Confidence working independently in a field-based setting. Passion for delivering outcome-driven, patient-centered care. Reliable transportation for home visits.
