This role offers an exciting opportunity to make a meaningful difference in the lives of individuals requiring geriatric, memory, or developmental disability support. As a vital member of our caregiving team, you will deliver personalized care with professionalism, empathy, and attention to detail, ensuring clients feel safe, comfortable, and respected in their own homes. Your bilingual skills will facilitate clear communication and foster trust with Polish-speaking clients and their families. Responsibilities Provide comprehensive personal care assistance including helping with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor patient condition through diligent observation of vital signs, behavior, and overall health status Assist with medication administration following medical instructions and maintaining accurate medical records in electronic health record (EHR) or electronic medical record (EMR) systems Support clients with mobility transfers using patient transfer methods like Hoyer lifts and other assistive devices while ensuring safety protocols are followed Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment in compliance with home healthcare regulations Document all patient interactions accurately while maintaining strict confidentiality under HIPAA guidelines Communicate effectively with clients and family members in Polish and English to ensure clarity of care plans and address any concerns promptly Requirements Fluent in Polish with excellent verbal and written communication skills Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification required; prior experience preferred Proven experience working with geriatric patients, individuals with dementia or Alzheimer's disease, developmental disabilities, or hospice/end-of-life care Knowledge of medical terminology, catheter care, wound management, and vital signs monitoring Familiarity with home health care assistance procedures including medication administration and patient transfer techniques Ability to lift heavy objects safely and perform physically demanding tasks as needed for patient support Experience working within home health agency settings or assisted living environments is highly desirable Knowledge of home & community care standards, medical documentation practices, and home health regulations CPR and First Aid certification required; Basic Life Support (BLS) training preferred Demonstrated ability to maintain patient confidentiality while providing compassionate care in accordance with HIPAA standards Join our team if you're ready to bring your energy and expertise into a role that truly makes a difference!
