The Hospice Aide works independently in patients' homes and facilities and must be dependable, compassionate, and comfortable traveling between multiple patient locations throughout the workday. A current Massachusetts CNA license is required. Reliable transportation and a valid driver's license are also required, as this position involves driving between patient locations. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility. Safely assist with patient transfers using appropriate techniques and equipment, including Hoyer lifts and other transfer devices when necessary. Provide compassionate personal care while maintaining each patient's dignity, privacy, and individual preferences. Provide catheter care and other patient care tasks within the scope of the Hospice Aide role and applicable regulations. Observe and report changes in the patient's condition, functioning, skin integrity, appetite, pain, or other concerns to the appropriate member of the hospice care team. Complete accurate and timely documentation in the Electronic Medical Record (EMR) following each patient visit. Provide supportive care to patients with Alzheimer's disease, dementia, memory impairment, developmental disabilities, and other special needs. Provide comfort-focused care to patients at the end of life and support families with compassion and respect. Maintain patient confidentiality in accordance with HIPAA requirements. Communicate effectively with patients, families, facility staff, and the interdisciplinary hospice team. Follow the patient's Plan of Care and communicate any changes or concerns to the appropriate clinical team member. Safely utilize patient care and transfer equipment to protect both the patient and caregiver from injury. Work independently while maintaining communication with the hospice team regarding patient needs and concerns. Travel between multiple patient locations throughout the workday. Qualifications & Skills Current, active Massachusetts CNA license required. Previous experience caring for geriatric patients in long-term care, nursing homes, assisted living facilities, home health, or hospice preferred. Experience providing hands-on personal care and assistance with ADLs. Experience with dementia, Alzheimer's, memory care, developmental disabilities, or other special care needs. Knowledge of basic patient care, medical terminology, and catheter care. Current CPR/BLS certification. Familiarity with Electronic Medical Record (EMR) systems and accurate documentation. Ability to safely perform patient transfers and use equipment such as Hoyer lifts and other mobility devices. Ability to perform the physical requirements of the position, including assisting with patient transfers and other hands-on care. Strong communication and interpersonal skills. Ability to work independently and demonstrate good judgment in the patient's home or facility setting. Compassionate, dependable, and respectful approach to patients and families. Valid driver's license and dependable transportation required. Ability to travel between multiple patient locations throughout the workday. Ability to maintain a dependable schedule and arrive at patient visits on time.
