Job Description
Maple Leaf Health Care Center — Manchester, NH Full-time | Field-based, Manchester and surrounding markets About the Role Maple Leaf Health Care Center is a skilled nursing and short-term rehabilitation center in Manchester, New Hampshire. We're hiring a Clinical Liaison to own our referral relationships across the Manchester market and serve as the bridge between our building and the hospitals, providers, and community partners who send us patients. This is a field role. You'll spend most of your week out in front of case managers, discharge planners, and physicians — not behind a desk. You'll be the first person a family meets when they're deciding where a loved one goes next, and the person our clinical team relies on to bring in the right admissions. What You'll Do Build and maintain referral relationships with hospital case management and discharge planning teams, physician practices, home health and hospice agencies, assisted living communities, and community partners Cover assigned accounts across the greater Manchester area, including Elliot Hospital, Catholic Medical Center, and regional referral sources. Review referrals for clinical and financial appropriateness, coordinate with the admissions team and clinical leadership on accept/decline decisions, and drive same-day turnaround Meet patients and families at bedside to explain our services, answer questions, and guide them through the transition to short-term rehab or long-term care Represent Maple Leaf at facility meetings, calls, community events, and educational in-services, attend external marketing events ALF's, Home Health, Senior Centers. Maintain accurate referral tracking, conversion data, and account activity Provide end-of-day census and referral reporting to the Administrator and department heads Partner with clinical leadership to identify service line opportunities and communicate our capabilities to referral sources What We're Looking For Valid driver's license and reliable transportation — travel is a core part of this role RN, LPN, or LSW preferred Experience in SNF admissions, hospital case management, long-term care, home health, hospice, or healthcare sales Ability to read a clinical chart well enough to assess whether we can safely and appropriately take the patient Comfort managing multiple referrals and competing priorities in a fast-paced environment Strong interpersonal presence — you're representing us to hospitals and to families on one of the harder days of their lives We recognize strong candidates don't always come from the same career path.
