As a Home Provider, you will help create that environment - you will work closely with our team of Host Home Coordinators to achieve this. Moriah's leadership team brings decades of care coordination and leadership experience; we believe in supporting host home providers fully.
What You Will Do:
Location:
Must reside in Iowa (currently Polk, Jasper, Dallas counties). Must own or rent your current home, must be your primary residence. If taking a member in need of mobility assistance, must have a home that is fully wheelchair accessible. As part of this job, you will be welcoming an individual with an intellectual disability to move into your home and live with you as part of your household. This is a full-time paid position. Moriah will help provide supports to you; we ask that our contractors provide a safe, welcoming, inclusive environment for our members. Each day might look a little bit different depending on the member you support; Moriah Home LLC provides an extensive member-family matching process that is geared towards the whole needs of the person supported, and your family and home dynamics. Overall you will provide support with:
  • Help with activities of daily living to include help with meal planning, cooking, cleaning, and budgeting personal finances.
  • Assisting with medication administration and medical appointments; Moriah Home will provide paid training for this.
  • Help your member access and participate in the community, build relationships, and experience a feeling of belonging.
  • Maintain high documentation and communication standards with Moriah Home LLC - training will be provided, comfort with basic computer functions is a must.
  • Assist with transportation in the community.
  • Some members may need help with behavioral support or help managing emotions; Moriah Home will provide ongoing training and resources to help.
  • Follow all aspects of the member's individual support plan
  • Help your member achieve their goals, dreams and aspirations while growing independence.

What We Provide:
Hands-on training - we will provide all training on the person's care plan, medical and behavioral needs, and all training(s) relevant to their care. We will provide training with documentation. Assistance and guidance with programing and licensing (State) requirements. Moriah Home is dedicated to providing on-going training and will dedicate a care coordinator to work one-on-one with you to achieve this. Moriah Home also provides round-the-clock 24/7 emergency support . Moriah Home believes strongly that if you provide the environment and care, we will provide the support. Requirements:
In addition to opening up your home to someone in need of belonging and support, host home contractors in the State of Iowa must:
  • Be 18 years of age or older
  • A current Iowa resident
  • Rent or own their own home, apartment, condo (must have a lease)
  • Be able to provide care or support that may be physically demanding with or without reasonable accommodations
  • Reliable, safe transportation with a valid driver's license and minimum vehicle insurance coverage
  • Complete documentation, attend required trainings, attend care plan meetings
  • Maintain confidentiality at all times
  • All providers/contractors and any adult that resides in the home (18 years or older) must be able to pass required abuse registry screening(s) and criminal background checks
  • Experience with people with disabilities (Intellectually disabled, brain injuries) preferred, but we will train!
