Job Description

PRIMARY RESPONSIBILITY
The activities assistance will assist the Assistant Director of Nursing with the daily activity program and all duties in accordance with BHI policy as well as state and federal requirements. Plan, organize, and execute a program of activities designed to meet the need of the individual residents. Prepare and post a calendar of upcoming events for use by staff, residents, family, and visitors. Oversee daily activities to ensure resident safety. Participate in performance improvement and utilization review programs Maintain daily record of activities Complete activity and social services assessment on all admissions along with the MDS according to schedule Coordinate decorations of the Nursing Facility according to holiday/season Schedule and attend care plan meetings according to Nursing Facility policy. Notify the resident, family, and other team members of the scheduled meetings. Encourage resident participation in all activities offered; both as a group and individually Maintain volunteer program per BHI policy. Schedule, supervise, and assign tasks to volunteers as needed. Prepare and review monthly newsletter. Attend monthly resident council meetings with AD. Ensure all documentation meets regulatory standards. Communicate regularly with resident families and entire care team. Lead group activities. Report and document resident's progress. Visits with residents on a one to one basis as directed. Reports to the Activities Director on residents responses and/or progress to specific approaches utilized and maintains appropriate documentation. Effectively manages the care of assigned patients using knowledge, skills, judgment and decision making skills for the specific area. Effectively carries out Activities role as assigned Assist with activities program for Swing Bed Patients Perform additional duties as assigned.
Qualifications:
High school diploma or equivalent GED Current Certified Nursing Assistant Licensure preferred
